Employer Self Report on Inpatient

For my employer, I have to self report that I was inpatient. Here's a copy of the report.
Memo: Self Report of Hospitalization
1. Ms. Xxxx, I would like to voluntarily self-report that I was hospitalized from 11 - 18 Sep 20 at the Xxxxx Memorial Hospital. This hospitalization was in care for my Major Depressive Disorder and Post Traumatic Stress Disorder (PTSD) that I received while in the Navy from May 1994 to Jan 2019. I entered the hospital voluntarily and was not under any legal issues to make this entry. I was held at the hospital entirely voluntarily, and could check myself out when I felt that the program had given me the perspective I needed to reenter society. This program was beneficial, and I am attending ongoing outpatient treatments at the hospital every week. I have fully returned to work, and this has not affected my performance.
2. This event should hold no bearing on my qualification to hold a national security clearance. I entered this program voluntarily and have saught resources that will be useful for me outside of the inpatient program.
3. If you have any questions about this hospitalization or previous ones, please contact me as soon as possible.

I got grief going on, on top my depression and anxiety and now I got to worry about somewhere to live since I can't afford living here since my mom passed. I did when I was working actually more to be in their face I think about the stigma..... Standing up to stigma however it was a bit more of a pain in the butt in hindsight.... I'm still glad that I did it though
If there is any problem with this you can just get the doctor to send a note stating that you are fine to return to work and have a security clearance... Big hug...

I was brought up short by the stigma in my college's reaction, especially because the leader of my tenure review was a fellow psychology professor. He was only acting vice president of the college at the time, and therefore hoping to be made permanent vice president. He had a reputation as a liberal at my college, and to become permanent vice president he had to win the support of conservatives on the faculty.
To do that, he staged an extra tenure committee hearing just to grill me as to my PTSD. I went to a lawyer before appearing before the hearing to see if it was even legal to do that and received his professional opinion that it was not. My lawyer advised, though, to attend the hearing and see if I got tenure in spite of it rather than sue.
It was arduous experience allowing that acting vice president to use grilling me before that committee about my PTSD to boost his conservative backing for his career's sake. I did get tenure, but I was never promoted again after 30 years of steady promotions. I agree with SunCloud: beware of feeding any facts to workplace superiors that could feed stigma for your PTSD.
