I just came back from my INR check and it's 3.4. The lab techs at my clinic have recently been allowed to make dosage decisions for warfarin patients, and mine today said to HOLD my dose (10 mgs) tonight and retest in a week.
My dosing is 7.5 mgs on M, W, F, Sat. and 10 mgs on Tues, Thurs & Sun.
I think she's over-correcting. I think we should have just done 7.5 tonight instead of holding the entire dose.
What are your thoughts? Have you had your dose held for a slightly high INR? I've been so happy being able to test once a month, and I think if she over-corrects it's going to put my INR into a whirlwind of ups & downs....not like I'll go against what the lab tech wants me to do, but I just want to hear what y'all think.

I am just wondering why they have you take 7.5mg one day and 10mg the next? It seems like a bit of a jump to me...When I have changes in my dose, it is usually only by 1mg per day, at the moment I am on 7/8/8mg every 3 days. (Although I've been on warfarin a year and a half and my INR still fluctuates frequently).

Just my experience.
My INR has been a little inconsistent since going off of warfarin for a colonoscopy back in June. Ever since, it just never got completely back on track with the same dosage that had kept me therapeutic for almost 2 years straight. Just never got back on track. So my latest dosage had been 13mgs 3 days a week and 14mgs 4 days a week, and that had me steady for about 8 weeks. Now my INR jumped to 3.1, which isn't that high, but my doc wants me to down to 13mgs 7 days a week. I think that's a total over correction. So I was thinking maybe just going 13mgs 4-5 days a week and 14mgs the other days of the week and just letting him know I made the change after testing in two weeks.
But, after thinking about it, I've been seeing my current doc for a couple of years, and he's been a champ at managing my warfarin in general, and so I'll go his route, see how it goes, and then we address it again if it gets mucked up.
I've totally accepted that my life is going to be a series of INR struggles and dosage adjustments, some of which may seem a little weird. My only request of my doc is that he doesn't kill me. So far so good.
I've currently got prescriptions for 5mg, 2mg, and 1mg pills. My INR has been up and down since March, so it gives me some flexibility in dosing.
Maybe, someday....
I do love my home INR tester. After years of waiting 24+ hours for INR results, getting them in five minutes seems a little like magic.
And I love my doc too. Really, all things considered, this whole INR situation is so manageable and so NOT stressful that I really don't worry too much about it at all any more. Getting the CoaguChek was a little bit like regaining some freedom from being a "sick" person. Fewer trips to the doctor's office. Fewer worries about when to test or where to test or whether my lab had standing orders or whether I could get extra tests if I'm sick or taking antibiotics or whatever. Just less to worry about altogether.
More time to spend on the rest of my life...
Anyhow, we're switching to just one day, Friday, that I'll do 10 mgs, and retest in 2 weeks to see what happens.
This whole thing is just one big crap shoot. I've never seen a more what appears to be unscientific science than warfarin management.
