Second, just because you tell your therapist something does not mean you have to tell your parents, if you're not ready, then you don't have to tell them yet, that's your decision, and you could always talk to your therapist about it.
Third, I don't know what will happen, because I just don't know. But I think that you should tell your therapist, but don't feel pressured just because I say to, it is really up to you, I hope you know that. The note thing is definitely a good idea for a way to tell your therapist. I hope that you handle this situation strong, even though it may be hard.

The Therapist can help you learn Coping Skills to deal with this. You can be on medications to make the voices go away. There are also meds that can help with the urges.
If you keep this to yourself and don't get help your just putting yourself and others around you in danger.
