By far the most common form of pulmonary embolism is a thromboembolism, which occurs when a blood clot, generally a venous thrombus, becomes dislodged from its site of formation and embolizes to the arterial blood supply of one of the lungs. Symptoms may include difficulty breathing, pain during breathing, and more rarely circulatory instability and death.

There are people that have switched back to Coumadin for various reasons. A majority of those patients have APS.
For me, after having been on Coumadin for about 20 years, I have been pleased with Xarelto so far (switched to it back in November 2012). Less bleeding issues when having that occasion fall on the Mountain Bike, less fatigue and no food interactions as far as Vitamin K. But, it's not for everyone.
