By far the most common form of pulmonary embolism is a thromboembolism, which occurs when a blood clot, generally a venous thrombus, becomes dislodged from its site of formation and embolizes to the arterial blood supply of one of the lungs. Symptoms may include difficulty breathing, pain during breathing, and more rarely circulatory instability and death.

I wanted to throw this out there for those of you who are considering switching doctors. The sooner, the better if you're going to do this.
At the start of the year, the new health care laws are supposed to go into effect and tons of people will have access to health insurance/care who didn't have doctors before. The problem is that there may not be enough doctors to care for everyone ... (Here's an article from the NY Times about it... http://www.nytimes.com/2012/07/29/health/policy/too-few-doctors-in-many-us-communities.html?_r=0)
If you're wanting to establish yourself with a new doctor, you should make the switch before 2014. After that, you may have difficulty finding anyone who is taking new patients or who has room in their schedule. For folks who are hoping to get health coverage at the beginning of 2014, you might try (and I'm making this up--no idea if it'll work) seeing if you can get an appointment set up now for AFTER you have insurance, just to get a foot in the door.
I've already had one doctor tell me that I needed to "hold my place" with my primary so I didn't wind up doctor-less. So, something to think about...

Also, if you plan on purchasing Individual (not coverage through an employer) coverage on a State or Federal Exchange, the plans will likely have a have limited network of providers. So yeah, it's just a matter of planning, finding out which docs are in a specific network and getting your ducks in a row.
