By far the most common form of pulmonary embolism is a thromboembolism, which occurs when a blood clot, generally a venous thrombus, becomes dislodged from its site of formation and embolizes to the arterial blood supply of one of the lungs. Symptoms may include difficulty breathing, pain during breathing, and more rarely circulatory instability and death.

I know World Thrombosis Day is coming up so I'm re-energized to try to spread awareness. I asked my doctor's office today if they would be willing to put up posters or include pamphlets in their examine rooms. They want me to bring in some samples and they will use them if they approve of them.
I was thinking that either stoptheclot.org or clotconnect.org used to have a tri-fold pamphlet that you could download and print, but I'm not finding it now. I found a book mark that can be downloaded and printed. That will work, but I was really hoping for a pamphlet. I also found a 1 page double-sided handout, but that wouldn't work very well as a poster.
I found some posters from World Thrombosis Day. They look great, but I would prefer something that isn't so focused on a specific day.
Please respond if you know of a link to materials that a doctors office might want to use either as a poster or a pamphlet.
If I don't find anything else, I think I'll print the book mark and the world thrombosis day poster.

I'm rethinking this now, and I think I will bring that document along with the others and let the doctor's office decide what they would find useful. If they have a patient with a clot, the more detailed document you linked will definitely be helpful for them.

I didn't read these materials so I can't speak to the information in them but it seems there are brochures out there. Also, I didn't look for anything with PEs.
