effects of TPA after saddle embolism

My mom passed out at home on Wednesday. She was rushed to the ER and was conscious. Her only symptom was a elevated heart beat. After blood work, chest X-Ray , and then CT they diagnosed a saddle embolism. They held off on TPA until the following day. 4 days later she is having latent bleeding in the brain. I thought the effects would be immediate and latent side effects would've passed by now.

Anyone have experience with TPA Effects?

Hi, I'm sorry about your mom. I had tPA and had no bleeding issues except for some significant hematomas on my chin, arm and hip, from where I'd hit the ground after I collapsed from my PEs.

The ER doc explained the tPA risks to me and I had to sign a consent to have tPA performed. I don't remember specifics, but I was removed from heparin in the ER for a while and then administered tPA and then monitored for probably for an hour or so for any bleeding while still in the ER. At some point after that I was put back on heparin, but I seem to recall this all happened, from start to finish, in a few hours until I was moved to the ICU. They continued to monitor me for bleeding closely once in the ICU for the rest of that day.

I don't know how old your mom is or if she has other health issues,but age and other medical issues may have increased her risk of bleeding. Also, if she's on heparin or warfarin now, so many days after the tPA, perhaps that's part of the reason for the bleeding?

Hope everything is ok and that her doc can give you some answers.

she has emergency surgery yesterday to remove the hemmorage in her skull. she is not going to be able to go on blood thinners for 1-2 months, so they installed a filter. she's doing better today. they may release her in 3-4 days. there is still some clot debris in her saddle artery. she's had 4 CT In less than a week. she loss use of her left hand due to the bleeding.

I hope she's on the road to recovery now, though it may be long.
