By far the most common form of pulmonary embolism is a thromboembolism, which occurs when a blood clot, generally a venous thrombus, becomes dislodged from its site of formation and embolizes to the arterial blood supply of one of the lungs. Symptoms may include difficulty breathing, pain during breathing, and more rarely circulatory instability and death.
"One lot of 2-mg warfarin tablets made by Zydus Pharmaceuticals is being voluntarily recalled by the company for being oversized [1].
The company announced the recall of lot #MM5767, warning that the oversized pills could lead to an increased pharmacological effect of the drug.
"As a result, patients would be more likely to develop bleeding as an adverse reaction, and in some patients that bleeding into a critical organ (mostly the central nervous system) could be fatal. The risk of bleeding is increased if overdosing is repeated continuously on a daily basis," a recall notice, issued by the FDA's Medwatch Safety Information and Adverse Event Reporting office, reads.
The company noted that it has not received any reports of adverse events or product complaints but is opting to recall this lot as a precautionary measure. A company spokesperson told heartwire that there were 960 bottles of pills in the affected lot, each containing 1000 tablets; the pills are typically dispensed to patients in smaller bottles.
The lot in question can be identified by its national drug code (NDC) #6838205310; it was distributed across the US to wholesalers/distributors, retailers, and mail-order providers from November 2012 to December 2012."

I've never posted my initials or first name here, as you and others have, so you must not have too much to do as 'the activist' for PEs.
DS has strongly suggested that everyone use screen names or nicknames in order to provide privacy. I did that, always being aware that anyone with nothing else to do and the strange desire to do it could find me and what I've been up to.

I do have to applaud you for that very accomplished bit of PR with smoke and mirrors. Perhaps you learned that as part of your self-reported importance as an 'advocate' for the groups you represent?
You've managed to do everything but kiss the ring (grin), brought up a couple of irrelevant issues, used the word 'sorry' and totally ignored the only thing I really objected to: the use of my real name without my permission on a public board that has done everything possible to encourage privacy for the members.
The 'lost soul' issue isn't important and may have been resolved at this point. Done.
Did you actually say you 'disapproved' of what skihillguy did? Did I read that correctly? I don't remember you ever telling him to 'give it a rest' which sounds like something you may have said to me, if memory and the saved threads I kept serve me correctly.
Were you the one that reported him? Yes or no. Or perhaps you just stood by and enjoyed the show.
