Ranay, there are alternatives to prednisone. They are called steroid sparing drugs. Two that come to mind are CellCept and Imuran. Research them. Given the extensive medical issues you need to consider, you MUST get to a neuro who specializes in MG who will work closely with your other specialists. There are other medical options, but your situation is different from most MGers. Your docs will come up with a good solution, so don't panic about absolutely having to take steroids. Most take it for a period of time. Some, like me, do not take it at all. Hope this puts your mind at rest.

If you have a good neuro who communicates with your transplant doctors, they will work out what is best, and then test it. The main thing is the communication or the neuro experienced with transplant patients too (probably not too common).

Are you taking a maintenance kidney rejection drug now that suppresses your immune system? Cellcept is often used to prevent transplant rejection and also is used regularly as an MG treatment, so that might be a good fit. However it is slow to take effect and does not always work.

Prednisone is preferred for early treatment of MG for folks without other problems as it does work faster than other pills. Cellcept and Imuran (the brand names) are regularly taken, but can take a year to help.

The other fast result treatments are called PLEX (a blood filtering treatment) and IVIG (a blood additive treatment). Both of those are more complicated and bothersome as they require you to spend hours and sometimes a few days per treatment. They are also very expensive (maybe $15-20,000) for a treatment that lasts a month or two. Insurance companies are not very enthused about having them as the first treatment because of that cost.

Since you're already taking prograf for your transplant, the transplant team should be able to come up with a combination that would help with that and mg.
As I'm sure you probably already know, the steroid sparing drugs can also be hard on kidneys.
It would be very important to get your transplant doctor and your neuro on the same page.

Thank you all for the information, my nephrologist said that prograf and myfortic are the same as Cellcept and Imuran, my doses may just have to be adjusted with. My endocrinologist is not a big fan of me going on prendisone so hopefully I can get him and my neuro to talk.
I really need to find a neurologist that specializes in MG because even though my neuro diagnosed me very quickly, she has never had a patient with MG, she is young, but her father has been neuro for 36 years and has had patients with it. I am seeing him next week.

In many cases, prednisone is a "bridge" drug since it takes effect rather quickly. In my case, I was put on prednisone to stabilize the condition. Then the doctor started me on cellcept, which can take a year or so to have complete effect. As I am tapering off the prednisone, the plan is that the cellcept will start helping. The prednisone really gave my glucose levels a rise, but the alternative in the short term was not to breath or chew or swallow. I am now down to 15 mg every other day, and so far so good. Just remember, other than your neurologist, most doctors don't understand MG and will immediately say no prednisone for diabetics. Your doctors need to talk to each other so your treatment plan is developed properly. Hang in there, sounds like you are scared. That is normal. Send me a message if you need anything.

Myfortic is the same as cellcept, but prograf is not the same as imuran, although they do serve the same purpose, dampening the immune response.
Hope you can get your medications arranged so you can treat all your conditions with little side effects.

Prograf is an effective treatment alternative for MG as well as an immunosupressant for your transplant. Prednisone would only ever be considered in your case if your MG symptoms were to flare and lead to a myasthenic crisis. Then it would likely be a relatively short course of therapy to help you regain control. Yours is a very complex case that would require collaboration with the specialists caring for you.
