I have yet to have a spot pop up on my breast area (knock on wood!) but I know all too well how painful the spots are every where else, and I could not imagine having to sit there while they squish your breasts in between those things to do the mammogram.
I would definitely let them know all about your HS there and that you are concerned about the pain and discomfort during the mammogram. Hopefully you won't have to deal with an ass hat during this experience.

Don't get me wrong it hurt so bad I almost passed out. I just wanted to do it. And I did. She said you could see all the scars and active ones clearly in the image and she'd write a long note to send with the record.
I found out that all of the charges were covered. If I get called back for more testing, which is very likely, or any other procedure they may have to do...it would all be completely covered. Wow, talk about Blessings...
