Hi,
I honestly think you're feeling wretched right now because you were overmedicated on PTU. Your FT4 is scraping the lowest end of the range and that explains everything you're feeling to me. Really. Hypothyroidism induced by overmedication feels absolutely awful, and that is why many of us call it "Hypo-Hell." Whenever I got overmedicated on methimazole, it was debilitating and it would take weeks being completely off of meds before I got better, sometimes, it took me about 6 weeks to climb back up to normal after being overmedicated (until i got a small script of T4 replacement, and then it took half the time.)
Right now, be gentle with yourself. You should start feeling better little by little each day.

I was a too much of a zombie to panic back then. I just sat and sat and didn't do anything. Like Hannah says, you've got to give yourself some time after being like overmedicated (but not quite like that for you because you were on very teensy weensy doses). It will get better.

You must admit that the ATD you did take, does work only too well for you.

Yes, MMZ, the PTU actually has done quite well for me- along with diet and lifestyle changes.... and when I started this journey, you were the first to reach out to me and calm my nerves... THANK YOU so much.

If you went and got a TT, make sure your doctor puts you on enough replacement med and does not undermedicate you. Otherwise you could continue having the same hypO symptoms.

Don't worry too much, just take one day at a time (I had to) and before you know it, your 'slight' hypO symptoms will ease up.

Okay, I met with a surgeon today to discuss a possible thyroidectomy and walked out with an appointment scheduled for surgery in a month. She did an ultrasound and found that I have a nodule on my left lobe that sits next to my trachea, practically on it, which is why I always feel this choking sensation when I lay back or from time to time... she also noticed a small nodule on my right lobe. She said she was totally confused as my left lobe looked normal but with a larger nodule and my right lobe is inflammed and much larger than the left with a tiny nodule. She thinks I may not have ever really truly had Graves and that my symptoms and hyperthyroid came from the nodule possibly. She labbed me again check for antibodies for both Graves and Hashi's and if the antibodies come back negative then she will do an uptake scan to see if the nodules are hot. We decided that I will definitely have my left lobe removed (so a partial thyroidectomy for certain) and depending on if I am positive for Graves with the antibodies then we will decide on a TT. I'll know by Tuesday which route we may go as she was fine with leaving it open for me to decide. I asked lots of questions and she didn't sugar coat a thing. She said a very large percentage of patients walk in her office convinced they want surgery and walk out with a change of heart once she discusses the complications that are possible. She does about 3 to 6 thyroidectomies a week and said the risks are higher in Graves patients. She said I'm a good candidate for a partial thyroidectomy and we'll decide on the TT next week once she sees my labs and whether or not i have a scan. I will be put back on my PTU sometime next week at a higher dose than I was on and Lugol's 8 days prior to surgery. She said she could not risk a thyroid storm in surgery and anything is possible. She said I will be absolutely miserable these next couple weeks on the PTU being hypo and that I'll be miserable coming out of surgery until we find the correct dose of synthroid for me. I have to admit that a TT scares the jeebies out of me but at the same time in my gut, I completely have no doubt that this is the right route for me. I have been a mess this past year, living in fear of the unknown, and feeling like tomorrow wont come. I've hated it but I've endure it. I've been extremely proactive in my care and will continue to be. She looked at my complete history from beginning to end and even ordered my cardiology reports and scans from when I had the thyroid storm to review during my appointment. She said I bounced around quite a bit but also responded well to PTU. She thinks I'm the ideal candidate for RAI. I told her "NO WAY"... not a chance in hell. I said if I had another storm, I wouldn't think twice and I'd opt for a TT. I hope and pray this is the right path for me and that I will be happier when it's done. She said I will be trading my goiter for a scar and weight gain and I smiled and said "i'm okay with that any day". I did get my Free T4 and TSH back today already and the Free T4 came back the same as it's been the last two labs but my TSH came back lower now at .39 since Tuesday..... and after being off PTU since Sunday. Does anyone know if your Free T's stabilize but your TSH drops and suppresses again..... what does that mean?

I think seeing a surgeon was a good decision for you. Yes you can have a hyper functioning nodule that is causing the problem if your TSI is negative or very low. I think the tsh is the last to change. The true sign of what your thyroid is doing is the free hormones. Right now you are still hypo. Hopefully if they do a partial TT it might solve your problem now that you know about the nodules.
