Self Banning- How to set up, does it work?

I would like to know of your experiences, those of you who have self-banned in CA, NV or other states.
What is the process, is it revokable after a period of years or is it for life?
How does it work and does it work?
I understand you wouldn't get any winnings paid from the cashier, but what about the ticket redemption booths?
Also what about hotels with casino's attached, are you banned from the whole property or just the casino floor, like kids are?

To me it seems the safest course to not relapse.
Also interested in hearing from those who have not self-banned, why not?
I understand if gambling in many venues such as online and lottery tickets, self banning isn't available, most here seem to have issues mainly with casino gambling.

I must admit, the word "shame" was probably out of context and not really what I meant, but leaving anyone flapping in the wind when serious implications could arise is just plain wrong. You're admitting a problem with a certain thing and that should be confidential- I do, however, see that if you violate the agreement, you leave yourself up to criminal prosecution in some states and that really is on you.
If you're staying on the straight and narrow, that should be private, IMO, or at the very least not published in the paper some day.

I self banned at two casinos in the area. I just self-banned myself tonight because I screwed up again. The other casino I banned myself in February and it seems to be working for me. I think it is a good thing to do. Both times now I have gone and talked to them at my lowest and filled out the form. It is good because I know my biggest problem is when I feel better I will want to go. If you are on the premise after banning yourself you will be arrested for trespassing. Good luck.
