I have also seen people do some creative things with their bank to curtail their withdrawals, one friend got her bank to lower her daily cash withdrawal from ATM to less than $20. Another friend had someone else set the PIN on his debit card and not tell him what it was, making it useless for cash withdrawals.
Have you tried EVERYTHING and ANYTHING to get between yourself and your gambling? That is what you will need to do to succeed. No one can do it for you, there is no silver bullet, but I for one don't believe that anyone is a lost cause unless they have decided that is true for them. If you think you can or think you can't, you probably right.
