At the same time, i think i do understand a little where you sister may have felt a little frustrated. Her outward appearance is still a part of her whole, and i guess i would hope that people close to me would notice differences. But i do think i would have felt better after your explanation.
I do often wonder why i seem to be different from so many others in this regard (not worrying much about looks). And i also think that if someone is relatively healthy and doing well on the inside, it will probably affect they outward appearance as well. Do you ever feel that you can sometimes just tell by the way someone looks, that maybe they're rather messed up inside? (Trying not to sound too judgmental there.)

At work they are SO obsessed with what you drive. Anytime someone gets a new car, blah blah blah blah ............ One day somebody said, "Hey stuck up! I waved at you and you never looked at me or waved back!" THAT is because I don't give a rat's ass what people drive, therefore have no idea what they are in, and when I'm driving (safety being the only exception) I IGNORE people. THEN when I got a different truck ......... OMG!!! The interrogation ....... ugh, people ............
I would have said the same thing to my sister. No, I would have lied and said "Oh wow! Nice!" You did the better thing.

I think your Sister got a little too pissy because you didn't answer her question "How could you not notice?" with the answer that SHE had in mind/wanted to hear.
That is a pet peeve of mine. Why ask a question if you are going to get pissy at the answer? Just make the statement instead.
