The superficial vein thrombosin is of much less concern than the deep vein thrombosis. I recommend that you talk to a doctor about this. He or she may recommend anticoagulation therapy. If the symptoms are unbearable or the vein looks bad cosmetically, then it can be removed or ablated (closed, e.g with radiofrequency). The thing is that as the superficial veins only transport 10% of the blood up the leg, while the deep veins transport around 90%, removing or closing these poses little harm. However, considering that you had DVT previously, then you need to discuss carefully treatment options with the doctor.
These symptoms do not fit with post thrombotic syndrome. The reason is that because the vein are located deep in the muscles, that if these are bulging due to poor blood retention, and causing fluid accumulation, this should cause general swelling and ithcing over a larger area rather than in a single location

It is odd that you're clotting even though you're active and on your feet a lot. I don't remember if you've been tested for the regular clotting disorders or not, but that'd be more questions to ask the hema. (Note that not all of the testing can be done with warfarin in your system.)
Geez... I really feel for you. Crazy that you've got this diagnosis despite having the right treatment. I hope you get some answers soon.
