explain symptoms, ask for testing (mri - xray - endo - blood..whatever they're passing out.) ask for pain treatment. when or if denied ask for referral to pain management physician or find one in network and repeat steps.
for more information on finding pain practitioners in your area google the american pain foundation (http://www.painfoundation.org/) read it and search. if money is an issue call all of them and see if any work on sliding scales.

please get help for yourself. no one else can do it for you! you'll always be your own most vested and best advocate.

I'm all for venting about pain and asking for advice but at some point you've got to take some steps to do something about it.
I am sorry I dont have a answer for you as I was a adult when my CP came into my life so I just took care of my self and not relayed on my mom taking me to the doctor or ER the only thing I can think of is go to who ever is in your life and ask for help if you have not before.
Do you tell her on a day or weekly basis that you are in pain and hurt and ask for her help? it is one of those situations that she may be so used to you saying it she no longer hears your cries.
If your caregiver is not listening to you or abusing you by refusing you health which I doubt but I am not sure then you need to find help from somewhere else. I thought also you had a baby and in a way I am glad if that is not true as you need to help you before you can help a baby.
