Topamax is an anticonvulsant that is used in the shotgun manner that they mostly are. It will only prevent migraine headaches or reduce the number of attacks but will not treat a headache that has already begun. As you are already suffering its probably not your best bet right now.
Imitrex on the other hand is designed explicitly for migraines and will only treat a headache that has already begun. It will not prevent headaches or reduce the number of attacks.
I would consider getting scripts for both if this is a common occurrence, but check with your Pdoc before using the Topamax as it is also used to treat BP.
If you are in that much pain you considered going to the ED see if your doc will give you a shot of something as long as you are not driving.

I was on Imitrix for years for migraines and Topamax as mood stabilizer for BP. Then taken off Imitrex as it messed with my already bad heart and now the topax is dual cross over drug.
Topax will make you shed the weight big time
After 5 years on Topamax, I slowly weaned myself off. What I discovered was that I have almost no memories of that period of time. They say it caused short term memory loss, which means while you are on it, there is not short term memory to convert to long term. LOL I can remember everything clearly before and after, and my memory has mostly recovered. I read Harry Potter books 1 & 2 before and 3-6 during. I reread them all before 7 came out, and I could remember the first books, but 3-6 were like I had never read them. I did lose 30 lbs, but regained it back.
I think I needed to be on Topamax for a while, but I would advise you to try lower doses to start, and see if it controls it. I have been off for 5 years and only had 3 migraines since. It was every time I took a pain killer, including after surgery when the nurses forced me to take it on an empty stomach.
I came to the conclusion that I had a damaged stomach lining, because it was always certain foods and meds triggered them. (Leaky gut) I don't take any pain killers anymore, because that seems to work. My stomach is improved. and I hope I never have a migraine again.
