It says social constructs, such as groups (friends, cliques, etc) either official or unorganized, exist for themselves autonomous of any individual within them. It states that human consciousness and recognition of these social objects cause them to exist and give them realism in the human mind.
That's basically it, as from what I can understand of the definition. It's hard to put it in any less academic terms than that. I don't know if I can "dumb it down" any more. Let me introduce you to my two good friends.

I am sure their catholic religion plays a part in their faiths and place in society but I have no Idea how to explain it........
Positivist theory: Everything that exists in the world is REAL and UNMOVABLE. For example, rocks just ARE because they ARE. Cancer is cancer because it IS. Love is LOVE- it is a "natural" human emotion that has always been the way it is. There are moral things, and amoral things- that is just the way "things are."
Idealism theory: Everything that exists in the world exists only because humans conceive of them and think of them. Without humans- nothing has any meaning. For example, people "invented" love, hatred, sexuality, the concept of work, etc. None of this stuff exists in the "natural" world. Extreme version of this theory claim that everything is "socially constructed"- including illness, sadness, etc.
Realism: So, the combination of the two looks like this: There are "real" objects in the world- there really are "abnormal cells" that cause cancer, there really are emotions, there really are rocks, etc- but these things only have meaning in the social world. Nothing in the world around us- while it exists (unlike in the idealism perspective) MEANS anything until humans have comprehended it and created structures around it.
