Stronger, I got a haircut a few months ago, after a decade of not one single professional hair cut. This is what I did. Went great :)
1. Picked a good salon with a good rep
2. Called the salon. Asked to speak to owner.
3. Asked her if she knew about trich/aka hair pulling disorder. She had. She was sympathetic. Bingo! I booked an appointment with HER.
4. When I showed up to the salon she was lovely. I was so scared and emotional but she made me feel relaxed. She asked what I wanted, I told her to do whatever she wanted, something that suited my face.
5. Ta da! Lovely hair cut.
So my advice is definitely to call before hand and discuss the fact that you have trich and are a bit sensitive about it. Or, if you can't do that, say you have a genetic hair condition that causes hair loss, and that you're a bit sensitive about it. This will almost ensure she/ he won't say something horrible/awkward to you in the salon.
I know the feeling though, of what you went through. Have spent many times in the bathroom cutting my hair very short to try and catch the bald areas up quicker.
Right now mine is not all grown in yet, and hiding under bandanas and hats right now, but in a few months it should look really good and I am looking forward to that very much.
