It isn't wise to mix patches with any other nicotine source. Very easy to OD on nicotine that way. Your hurting heart may be a sign you're getting too much. Not all doctors are savvy on the details of quitting smoking, even P-docs.
You don't "bust through" cravings. You wait them out. Sure, they feel like hell and make you wonder if you'll survive, but you must keep this in mind at all times - they are signs of healing and no damage is being done. Repeat it out loud as a mantra, if you have to, but don't give in to the addiction.
As you break the mental associations between smoking and various events/emotions/activities, the cravings will diminish in frequency and severity. This is a long process, it won't happen overnight, but they do subside. Patience and presence of mind are your greatest allies right now.
