As for using it off label as a med to stop urges, that's a new market and has not been utilized very long, I doubt you'll find a lot of people who have much experience with it yet. When I was heavy into cutting, my Pdoc used Lithium to help calm my urges and it worked quite well, that with a combination of DBT, helped me get the urges to cut under control. I also learned how to use regular things to take my mind off of urges when they hit. I'm a big Journal-er, I Zentangle, and I color Mandala's and Florals. There are many different ways to take your mind off of the urge to self harm, many different coping skills to learn, ways to learn to use the different lobes of your mind, i.e. right or left at different times to help curb the urges/feelings your having at the time.

The point is you have to decide if you want to stop doing this or not and you have to work hard to stop, nobody else can do it for you, there is no medication that will make the urges stop completely, those don't exist yet. I've got 35 years in self-injury, 28 years in BiPolar, a Ph.D. in Clinical Psychology that I don't use professionally these days due to increased medical illnesses/complications, but I keep my licensure current and I keep up with colleagues and I'm a Research Geek. My Pdoc of 10 years is one of the foremost "Medication Management" Specialists on the East Coast, he'd love it if there were new meds that could help patients curb their desires to self harm and cut, but so far there are not any big drug trials going on in that area. A few psych meds are used, but they are intended to help with additional therapies like DBT and Talk Therapy.
To stop you have to be ready and willing to do whatever it takes to do so.
Take care, and if you do start using the Naltrexone make sure you tell your other Dr.'s that treat you for your medical illnesses that your on it, they need to know.
