I have used both extended release and regular. I have found that regular works better for me. I take 50mg in am and pm and found that this works better than 200mg xr/once a day, but everyone's body is so different. I'd give it a try and than you'd know if it helps you to have a more stable blood level.

That's pretty much what my doc said. She wants me to give it a couple weeks at least to know for sure.

Hi Lisa - I'm in your shoes sort of -- got a script for Tramadol recently, said to take 1 50mg twice a day as needed. Well one 50mg didn't do squat - so I am taking two 50mg first thing in the a.m. - and one or two by the afternoon.

I don't know what my doc will say about this - I plan to call him next week if my pain is not subsiding at all.

I don't take tramadol, but I do take an extended release version of morphine called Avinza. It's been wonderful. I, like you, was disappointed when I had to move to an extended release drug, because I was really hoping there would be days when I wouldn't need any pain medicine at all! I kept a little journal for a few months, just jotting down what I was taking and my symptoms, and I saw that it had been months since I had any days when I really didn't need any pain medicine. The extended release dose provides a constant baseline. I didn't get to a point where I don't need any of the immediate release drug (hydrocodone), but I was able to cut down quite a bit. I hope the long- acting tramadol is an improvement for you.

Thank you for the replies. The pharmacy said it should be in next week, so we'll see how it goes. Scarlett- my script said 1-2 every 4-6 hours so I know you aren't taking an unsafe dosage at least. And neither was I, I'm not sure why my doc felt the need to lecture me. She told me the upper limit is 800 mg. and I'm only taking 200-250, not even close.

That's 800 mg. in a 24 hr. period btw. In case anyone wonders. Personally I don't generally wake up enough during the night to get up and take any (thankfully) so I only take it during the day.

My ER came in this morning and my hubby picked it up for me. I took my first dose about a half hour ago. So far, I feel about the same as I usually do within a half hour of taking the regular stuff. Low pain level and a bit loopy. Hopefully the longer I'm on it, the less loopy I will be. The question is how effective the pain relief will be as I adjust.

Just a little update here. I've been on the ER for a bit now. The first week I still needed one 50 mg. pill in the evenings, but since that first week I haven't needed a single one. My pain level has been very low. But it's too early for me to really say if it's the ER Tramadol being really effective for me or not. I also feel better in general right now- I'm not so fatigued and I feel pretty good. So I could just be having a good spell anyway, they do happen. Time will tell.
