I'm so sorry your having these problems! I know how hard it can be not knowing what is going on. I would do research on the computer and just keep communicating with your Doctor until you get some definate answers! This site is great for support! Hang in there!
These symptoms certainly can be RA. Fatigue can be terrible with RA as well as lupus, and also very low vitamin D contributes to fatigue as well.
I can see your concern with lupus re. the red spots/rash. Has your doctor run an ANA test? This is a non-specific lab, but one that is frequently elevated in lupus. If your ANA is positive, there are "sub-tests" that can be done to test specifically for lupus (I believe it is the anti-DNA test).
However, as far as the RF goes: 20-30% of those with RA (depending on what source you read) never test positive for RF. Many here on this list (including me) are "seronegative".

MTX can take several months to kick in.
Also you say you have negative eye issues on Prednisone. Did your Rheumy tell you Plaquenil can do so, too? On Plaquenil I was told to see the Opthamologist every six months for a check-up.
The RA fatigue isnhorrible. Hope that you get some relief soon.
