Are you seeing a Movement Disorder Specialist Neurologists? I know we ask that a lot on here - but it is very very important that you regularly see one or that the doctor you are seeing consults with one about your treatment. These Specialists are specifically trained to deal with Parkinson's and other movement disorders.
Good Luck - I hope you can get some help with these issues you are having. The breathing problem especially sounds scary.

I have PD and heel pain, I've been told mine is common and called plantar fascitis. For me the fascia is shortening in both hands and feet, so walking tends to be tight, not rolling through the foot properly as the heel is tender. A visit to a chiropractor or your GP as already mentioned may well be worth a visit to try and ease the pain. Things I have found to improve things are a foot exerciser which you roll your foot over along the floor, pilates and massage. Best of luck.
