Paranoia is a disturbed thought process characterized by excessive anxiety or fear, often to the point of irrationality and delusion. It is often associated with psychotic illnesses, sometimes schizophrenia, although attenuated features may be present in other primarily non-psychotic diagnoses, such as paranoid personality disorder and obsessive compulsive disorder.
We can't control everything that our brains think, and I have had some thoughts of hurting people I love even though I never would hurt them and I don't really want to. It's not a choice for me to think these things, they just randomly pop into my head. That is also a symptom of obsessive compulsive disorder.
I really suggest seeing a psychiatrist to help with your situation, as you sound like you are suffering from hallucinations (apparitions telling you to do certain things). Medication and therapy can greatly help with this. :)
