Obsessive-compulsive disorder (OCD) is a psychiatric disorder, more specifically, an anxiety disorder. OCD is manifested in a variety of forms, but is most commonly characterized by a subject's obsessive (repetitive, distressing, intrusive) thoughts and related compulsions (tasks or rituals) which attempt to neutralize the obsessions.

The surgery helps to remove the "cause" of the OCD, but after years of having OCD, there are a lot of learned thought patterns. It is not an overnight fix.
I have read about the surgery in the past, and I don't think it necessarily involves a substantial amount of interference with the majority of the brain, but before going forward with any plans for it, you should always ask for a second (and a third!) opinion (that goes for any serious procedure that isn't a common reccommendation). I wouldn't rule it out, but really, get seen by as many specialists as you can, and see if you can find anyone who has actually undergone the procedure, and hear what they have to say.

I just was reading about it prior
to reading this post. There are a few areas of the brain involved in
OCD, one of the primary ones being
the caudate nucleus. From the reading that I have done, Gamma rays
are what are used for the surgery.
On the website I just visited (I
Googled "Cingulotomy patients'
stories), the information about the
surgery itself said that the outcome
is usually an improvement of from
25 - 70%, depending on the case.
The site also said that doctors
would only consider surgery after
the person had tried at least one
or more Cognitive Behavior Therapy
treatments and tried different SSRI's at full dosage.

I am trying a 12 step group for OCD. I also try to walk about 5 miles daily.I am trying really hard to stay positive,refocus and work the steps. I think I am doing better.
