It looks just like a super super large hair roller--dense foam over a hard plastic cylinder.
I love that it has a soft and puffy top layer, but has a hard durable center.
That scratchy feeling is from toxins and will go away. Sometimes it would even set off burning. I would just ice my back. That only happened 2 or 3 times in the beginning.
Take pics of your back now, and see if things change as areas start to feel better. My dr was shocked at my before and after pics. I was so off balance it was no wonder I hurt the way I did.
If you have TrPs in your neck, I would suggest getting another roller (inexpensive one) to balance your neck on when you start using it on the floor. I made a mistake using it without and made the triggers in my neck worse.
I also make use of furniture to get in between so I don't need to stress my shoulders/arms by trying to support all my weight with my hands on the ground. I've needed to get quite creative in ways like that to not aggravate other areas.
I also keep it next to my bed at night incase I wake up with hip pain.
