If the clinic/office calls you back and apologizes and seems truly sorry for the problem, then I would let it go. If not, they deserve a written complaint sent to the doctor, the clinic administrator, and the state health department as well as the Better Business Bureau if you are in a bigger town. Mix ups can happen, but how they deal with it will let you know if they are trustworthy in the future.
The medical world has many very good people who often are overworked and that does cause problems. There are also doctors and clinics who are just not very good, and trying to figure out which it is, is very important.
