Myasthenia Gravis Support Group
Myasthenia gravis (MG) is a neuromuscular disease leading to fluctuating muscle weakness and fatiguability. The hallmark of myasthenia gravis is muscle weakness that increases during periods of activity and improves after periods of rest. Although myasthenia gravis may affect any voluntary muscle, muscles that control eye and eyelid movement, facial expression, and...

200mg is a stiff dose but in a crisis, it may be considered worthwhile. When I was in a coma, my dose was 100mg EVERY day. I'm not sure there is a direct translation here between my dosing/schedule and the one your doc is suggesting, but every other day dosing is not uncommon. It allows the adrenals to come back to life on the "off day."
Crisis implies a severe spike in antibodies and that requires serious medication. Many folks in this group and their docs seem reluctant to use prednisone, let alone use it in high doses. I definitely don't agree with that position and since there's so much "witchcraft" used in the treatment of MG, this advice must come as a shock to those accustomed to the alternative therapies so many use.

Be very careful about how they taper you off a high dose of prednisone. I was sent home and told to go back to my 20 mg per day. Two days later I was really in crisis.
It was back to the hospital and back on the solumedrol. Four days later they started me on a taper. The first 3 days was at 60 mg of prednisone per day. Then 3 days at 40, 3 days at 30 and then back to the 20 mg.

I believe my total prednisone for that incident and that day combined was either 120mg prednisone or 160mg prednisone plus the IV steroid. I continued to take 40 mg daily after that and am now tapering.
I am now on more moderate doses because they don't really seem to help any less and I have suffered many of the more serious side effects of pred.My disease is one of the rare cases that is pretty steroid resistant though unfortunately so the high dose regiment may be different for others.

I am finally at 10 mg every day and we're tapering very very slowly by 1mg per month. If you get off prednisone too fast, you might relapse if other meds have not kicked in yet. Prednisone does work to keep you going! Take Care of yourself.
