I think I'd call the health department in your area and report the manicure place. I totally understand you wanting to hurt the culprit!
Your doc should not be thinking it's anything, he should have cultured it immediately and "know" what it is. The only way to know which antibiotic will work is to culture and do a sensitivity test.
If he not done these things? If not, find a new doc right away. He should not be guessing about this.
