Migraine is a neurological disease, of which the most common symptom is an intense and disabling episodic headache. Migraine headaches are usually characterized by severe pain on one or both sides of the head and are often accompanied by photophobia (hypersensitivity to light), phonophobia (hypersensitivity to sound) and nausea.

I've been suffering with migraine for 32 years, so I know what you are going through. I hope you are successful this time around in your appeal.
On another note, my mom used to have severe migraines on a regular basis just like you. After years of trying to figure the migraines out, she came across a blood pressure medication which cut away or greatly reduced her migraines.
I don't know if either of these ideas will help you, but I share them just in case. I hope you find relief - headaches are no fun.
