One of the most important keys to better sleeping is simply setting and keeping a consistent schedule that is supportive of good sleep. This usually means getting up at the same time each morning consistently and not allowing yourself to sleep in no matter how tired you may feel in the morning.
Bedtime should be determined by how much time you want to allow for sleep. You can use sleep timing techniques to build prior wakefulness, and in so doing you increase your sleep drive. This tends to support better sleep.
Having a relaxing wind-down routine before bed helps, and that can include a relaxing warm bath as you've described. Diet, exercise, lifestyle, and your bedroom environment all play an important role in good sleep.
Since you already are on a number of medications, you should discuss this with your doctor. You might ask for a referral to a sleep specialist or counselor who knows about CBT for insomnia.
CBT enables you to comprehensively address the nonmedical issues that could be contributing to your sleeplessness. This is what I used, and it works.
