Based on your description, it wouldn't be at all surprising if what you really have is insomnia that is secondary to the other significant medical and psychiatric conditions you describe, plus the various medications possibly being a contributing factor.
To best sort through all this, and to arrive at a unified medical plan to treat it, my suggestion would be to see a medical doctor who specializes in sleep, if you are able.
Your case is quite complicated, but I suspect it might simply boil down to finding the true root sources for whatever is interfering with sleep -- treat those -- and your sleep should tend to improve.
And be realistic. No one ever sleeps again like a baby. Increasingly fragmented sleep is just a normal consequence of aging. Happens to us all. Part of your solution may be to accept this and try to release some of the worry/anxiety/stress you associate with the idea of sleep.
I am not looking anymore. My last three specialists all said the same. The damage is done and it is more likely that medical science will take 10+ years to figure our enough to know what I have than not. After that I stopped looking.
