Yes, if she's going out the door anyway, time to take her off your phone plan, or cancel it all together and get your own individual plan. Make her pay for her own infidelity. While you're at it, make sure you set up your own bank account that she is not on, if you don't already have one, and divide the money accordingly. Otherwise, you could find it drained.
Get her off your plan ASAP.

She should have undergone a psychological assessment prior to the gastric bypass surgery.
As her to make an appointment with that psychologist and touch base about what all is going on with her. It may just do her some good, because it sounds like she is starting to lose control.
In the meantime, cut off her phone and give her a phone allowance till the judge signs off on the terms of your divorce.
Who have out outed her affair to? It might be time to let her parents know gently that you love her and them, and that you hope to be the best ex-son in law you can be.
Ask her sister for help.
The goal is not to be vindictive, and don't get your hopes up too much, but sometime shining some sunshine on someone's behavior helps it die!
Any change in psych meds with the surgery? Did she go on or off any meds after the surgery? I might even contact her doc and leave a message about concern over erratic behavior. They will not talk with you because of HIPPA laws, but they will receive your message never the less.
She just sounds to me like something bipolar or manic has clicked in her post surgery.
She is obviously making her choices and she needs to know that she can deal with the effects of these choices
