Ice will give you bumps, and honestly if the alcohol in your skin is dry - have DH fan the area after wiping it - the needle you barely feel it.
Other than what Chefgrrl said another key thing is to make sure you stay within the circle and don't go too high. I too have junk in my trunk! Good luck!
They also say to walk around afterwards to get a little movement into the muscle. I don't remember the exact explanation from my nurse but I remembered that instruction.
I alway had my nurse use a sharpie pen and draw the zone where it was acceptable to get the shot. I kept re-drawing the circles each day for a while until I got the hang of where to do them.
Don't panic if you start to bleed, just pull out the syringe and put on a new needle and try again.

I too have junk in the trunk and my nurse used a sharpy on my butt yesterday so he shouldn't miss the spot.
Not to sure about the blood return but I'll let DH deal with that and I'm sure DH will be happy to massage my butt :)
Nelly, my instructions are to leave the needle in for 45 secondes after the shot. That's so much longer than you but you are right way better than the cream and so worth it if I get a BFP that actually sticks.
