I had to have:
Psych eval
EKG (which then led to a stress test)
Ultrasound (which led to a Endoscopic one)
Sleep Apnea test
PT session
Pre Op Physical
And numerous little other things, I've probably forgotten
Primary Care Evaluation and he had to provide a letter of medical necessity to get things rolling
Psychiatric evaluation
Upper GI
Chest Xray
Blood Work
Gastroenterologist Evaluation
Upper Endoscopy
Pulmanary Evalaution
Sleep Study
Cardiologist Evalaution (scheduled for next week)
I have also had 4 months of nutrition counseling so far.
I had to attend at least one support group meeting faciliated by my surgeon's office
I had to attend a pre surgery class given by the nurses at my surgeon's office
I am scheduled for my surgery for 9/12 and am just waiting final approval from my insurance company! Keeping my fingers crossed!!
I too have noticed huge variences in what each program requires pre surgery as well as after surgery care and nutrition.

This was in addition to the pre-op requirements by my surgeon and insurance company. All patients were required to consult with a nutritionist and a psychologist. The other consults and testing were based on personal and family medical histories.
