keep after your Doc if they cant get to the bottom of it send you to specialists (Rheumatologists, or Neurologist) I had a friend who had a pinched nerve in his back and it caused extreme pain in his foot. He had surgery and told me the surgery pain was slight compared to the foot pain.
I wish you well and keep pushing for an answer
It is so frustrating to go for test after test and not know what is wrong. All of us with FM can sympathize with you on that. But it may not turn out to be FM. There are many diseases that can mimic FM; Lyme, MS, RA, Celiac, Lupus, to name a few.
Good luck, I hope you find out what is wrong so at least you know.
Along with Fibro, you could possibly have myofascial pain syndrome, chronic fatigue and/or Ankylosing Spondylitis. Here is a good article and diagram from my doctor's blog. It shows the tender points of AS. There is a few things they can check for for AS such as a gene called HLA-B27, sedimentation rate and C-Reactive protein.

CoEnzymeQ10 is supposed to be helpful for headaches as is 5-HTP. 5-HTP is a natural anti-depressant so shouldn't be taken with other anti-depressants.
For energy and muscle repair, try D-Ribose powder, Hemp Hearts, B12 and other B vitamins and for pain...SAMe, vitamin D, and/or boswellia.
I take 5 mgs of low dose Naltrexone (Revia) a night. It's an anti-inflammatory and anti-viral.
