What I've found that has made all the difference for me is using a two-headed percussion massager like the one at the link below:

I think using this is the reason I've had so much success with the trigger point massage as I don't have to look up specific trigger points for pain patterns, I just start massaging the general area where the pain is and if there's an active trigger point, I usually hit it and am able to work it. If there are other active trigger points, I hit them as I'm massaging the painful area and it seems to keep them under control.
Another great thing about the percussion massager is that I often wake up during the night with the trigger points on one side of my body almost locked up. I'm able to lay on the couch and use it loosen everything up in a few minutes.
