I have everything you have except the rash. Have you had your CPK level checked? What about an EMG?
I would ask your doctor to do a blood test to check your vitamin D, B, and Magnesium levels.
I had an ER doctor check my magnesium level back in febuary or march and it came back really low. He gave me a prescription for magnesium for a few days and now a take a supplement with calcium/magnesium/zinc and my pain level has gone way down. I take it in the early evening because it also works as a sleep aid.

Muscle Relaxers Several physicians recommend using natural muscle relaxers to get the body in the mood for going to sleep. Dr. Rosenbaum recommends calcium (600-800 mg)/magnesium (300-500mg).
For the rash if your doctor isn't much help you might ask to see a dermatoligist.
Alot of us have both fibro (FM) and CFS
I think they are going to find this link to the xmrv virus in alot of us but it's still in the research phase.

The pain in the top of your feet could very easily be myofascial pain where the muscles get knots in them that refer pain elsewhere. The knots are called trigger points and have to be broken up in order for the pain to subside. Fortunately you can learn to do trigger point massage on yourself with a bit of time and effort.

A couple of other good sites are triggerpointbook.com and triggerpoints.net. The trigger point therapy workbook is well worth the cost if you have myofascial pain which goes hand-in-hand with fibro. You can get it cheaper through Amazon.com.
I find using an electric massager works better than a manual one as I can massage my body focusing on any spot that hurts [an active trigger point]. Since learning trigger point massage 5 years ago, I've gone from being in one degree or another of pain 24/7 to being pretty much painfree about 85% of the time.
they have one for restless leg syndrome and I've used it and used it on my husbands's leg when his twitching would keep me awake.
Average is 15.00 per bottle, 10.00 when you wait for a sale.
Have also used the oils for joint aches and headaches. May be worth a try for you.
