You husband will be the "principal" since he is the one giving you the POA and the permission to carry out financial transactions for him. For his address you will use the prison address.
You will use the forms of the state in which you are living, since that is the state which will be enforcing the POA.
The document portion that you download or create from the site will be a generic document that will cover 99% of most people needs. I am relatively sure that you and your husband do not have several million dollars stashed away in liquid assets or savings, so the online document will work fine.
There is no need to spend any money as this can be done for free. If someone mentions that you need to spend money for this, they just want to get into your pocketbook.
Once you fill out your portion, send it back to your husband. He can sign it, have it notarized either in the case managers office or at the law library and send it back to you. You can then have your signature notarized on the last or next to last page.
After you have both notarized both signatures, you are ready to go.
You can then use it whenever you do business for your husband.
Every institution where you do business on behalf of your husband will want a copy of the POA on file, so it would be advisable to make several copies for yourself.
It would be a good idea to call the prison, as some of these facilities are cranky about how POAs are created. Get a good idea of the procedure they use in prison.
