You are probably right that there are going to be both parts of the wedding process where his absence would be noticed or things like pictures of the wedding party where they pull in this side of the family or that, etc. where it will be conspicuous.
In all seriousness, though, your ceremony and event needs to be 99.5% and this drama (not created by you) needs to be a very minor thing compared to the event. You are being joined to someone, not your brother. I think if you and the folks around you agree not to make a big deal of it, it won't be. Probably a "he couldn't make it" and following up with any further questions with "I'm disappointed but I know he wanted to be here and can't really get into all the details" followed by a topic change would stop most people.
