I made an appointment with a doctor for my depression and anxiety. This is the first time I have ever attempted to get any sort of professional help. Could anyone tell me what to expect when I go? I feel a little nervous.
I was also thinking about writing about all of my symptoms and what I go through. Does anyone think that is a good idea? I am supposed to be applying for medicaid and disability so I don't wan them think I am faking anything. I just need the medicaid so I can get professional help so I can get back to work and support myself.

You will probably see a social worker, and then the pdoc. I am a little nervous about this new place today so I can imagine how you feel.
I think a list would be a good thing. The pdoc will ask you questions also. I am glad you have decided to go see a doctor. Hope it all works out for you.
Just be open and honest- keep an open mind. Keeping a journal is a great thing...i wish i would do it regularly myself. When i have kept a journal in the past, i would let my therapist read it- that way she knew what kinds of issues i truly needed addressed.
Hope all goes well, keep your mind open to new possibilities!
