OK, can you get somebody else to do another cleaning on the apt? If not, I'd say just try to ignore that it's not quite up to your standards and move in. Because the most important thing right now seems to be that you move. And then you can clean it when your energy level comes up.
You are absolutely not a failure. You're in a PhD program! Even if you quit school (which I hope you won't), you'd still be far ahead of most of us who never even got to that point.
If you have trouble even showering, I'd say the meds you're on aren't working very well. Some are more stimulating (you probably know that), so see if you can get on something that can give you more ooomph (that's a technical term).
All in all, I think the bad relationship with the roommate is the most pressing issue, so either get another cleaning of the apt. or just move into it as is. And if you can, hire movers to make it easier to do.

I wanted you to know that I have depression and anxiety. I received my Masters Degree in Social Work last year. Now, lately, I have been having issues with doing the simple things too, like getting into the shower, sometimes even getting off the couch to get something to drink.
