HI, all! I made my first ever request for a copy of my latest PFT test. The receptionist first said I would have to get it from the Pulmonologist. I had already called them, and they said I had to get it from the requesting physician. I told her what they said, and she said I don't think that is right! So I asked her to check. She came back to the phone, and said we have to charge a $25 handling fee and $5 per page. At that point, I kind of lost it and said I don't think so! These are my records, that just happen to be on file at this office! And I am entitled to a copy of the entire file if I want it! She checked again, and came back and asked if I wanted to pick it up? I just said please mail. Have any of you had a problem with this? Was I out of line?

If they are your own records, it's your right to access them.
If tests were requested by a Dr associated with a hospital or large clinic, they will have a medical records department. There is likely a form, however many records are now electronic as are requests.
You can generally request a date range or specific types of records. I've had to request my mother's records and accompanied my request with a copy of her power of attorney for healthcare and have never had a problem.
