Codependency Support Group
Codependency is defined as someone who exhibits too much, and often inappropriate, caring for another person's struggles. A codependent person may try to change, or feel shame about their most private thoughts and feelings if they conflict with the other person's struggles. If you are on a journey towards self-love, this support group is for you. Join us and find others...

To me, it's very telling... Are you feeling rejection? Abandonment? Are you feeling unimportant? insignificant? Anxiety? Depressed? (Meltdown has many definitions)
What is this "triggering" for you when she cancels an appointment with you? Is this bringing up a bad memory? A pattern that you grew up with?
Meltdowns don't just happen. They are the result of an emotional experience, which is caused by an internal reaction to something, someone, some place. But we cannot keep avoiding people, places or things. Because we bring our issues with us. So I really recommend that you honestly discuss this with your therapist. Because you'll only get out of therapy what you put into it. If you're afraid of the "confrontation" -- tell her that, too.
Good Luck.
