You mentioned you're covered by Medicaid. Organizations that administer Medicaid plans for individual states are under an obligation to provide adequate access to any necessary kind of specialist. Obviously they do not always do a good job of that, but there is a recourse. You could start by calling your insurance directly, and being persistent. If the first person you speak to can't help you find a specialist (from what you're saying, a pain specialist, neurologist or rheumatologist might all be appropriate), ask for a manager. You can also google "medicaid ombudsman" and your state. That will get you info on how to contact the state directly if your plan isn't able to locate a specialist for you. I don't know where you're located, but university hospitals often have good pain programs and might be worth a try even if it means a long trip from home.
