A neurosurgeon or a neurologist would be able to diagnose these problems and pinpoint exactly what needs to be worked on.
A lot of times there is scar tissue from chronic inflammation
I've fractured my back, broke 2 Vertebrae, the breaks healed well and I have no residual pain from that, but do get lower back/tail bone area pain from Peripheral Neuropathy. So its not always clear when your back is hurting where it's actually radiating from.
Having current images read by a Neuro or Spine Doc would give you the best answer without guesswork.
