I thought to myself, "I can't just let this go. The insurance company will think the doctor is a complete idiot." So I clipped a note to the file and sent it back to the doc, saying, "It's news to me that a spastic quad is capable of walking with a scissors-gait or any other kind of gait. Do you want to re-think this one?"
He wrote back: "Maybe you should be seeing patients and I should be typing reports. Thanks for catching that one!"
Most of the silly things in the original post were the result of having to dictate the same stuff over and over. There are certain set phrases the doctors are expected to use. After a while, the brain checks out and the mouth goes on autopilot. Then you get a transcriber who either doesn't realize that the doctor has made a mistake or doesn't care enough to correct it.
Together, they can come out with some GREAT material!
Shame on me! But I also feel bad for the poor patient whose knee disappeared! :)
