As for where to go with your testing.. have you looked at Lyme? Sometimes that mimics CFS. I would also rule out autonomic problems. I've heard some people discussing mold infections as the source of their fatigue, but don't have a lot of information about that. These are just a few of the avenues you could explore.
One doctor I spoke to said stomach viruses/bacteria could sometimes cause severe fatigue, so it's good to have extensive testing done to rule out issues with that. Just some ideas.
