It does sound like you have sole custody thought not legally and entirely not fair to you but you have to protect your daughter.
I'd end the visits for the time being. Don't make her a hostage to his mental health issues.
I would ask her why she would prefer to stay with you in a way that is random and not right at the time it is an "issue". And then communicate to your ex and say it's because of X, Y and Z. If you don't change these situations/behaviors, our child will not be comfortable being around you overnight. And then he will have to figure out how to tackle those issues. It's in his court.
And I'm sure you're thinking he will never do xyz....well, then he won't. Then he will not see his daughter and lose out on a great relationship. You can present him with the why she doesn't want to visit with him - in a non-blaming way (even though we know it's his fault). We all instantly get defensive when we feel blamed. So that's what I would do and have done. It works or doesn't. No skin off your back as long as you do your "part" by communicating to/with them both.
